USA-Valero's Texas City boiler had history of problems

A giant industrial boiler that exploded late Friday, killing a worker at Valero Energy Corp's (VLO.N) Texas City, Texas, refinery had a history of problems since it was installed in 2006, said an attorney for a worker injured in the blast.
 
Fireworks blamed for deadly blast

MORE than 100 people were killed in an explosion at a cafe in western Russia yesterday that was possibly caused by fireworks. Officials in the city of Perm ruled out the likelihood of a terrorist attack.
Another 140 people had been injured in the blast and resulting fire, a spokesman from the regional emergency situations ministry said.

A tragic article which highlights the dangers of pedestrians being in close  prximity to forklifts.

A man was killed when he was hit in the head by a forklift shortly before midnight on Monday, November 23, 2009. Jesus Hernandez-Blas was working in an industrial park located at 10111 Anderson Avenue in southwest suburban Chicago Ridge, when he was fatally injured, as reported by the Chicago Sun-Times.
