Workplace Safety - Capture and Act on The Near Miss

Most people involved with workplace safety are very aware of the iceberg theory.  
Its very simple. For every recorded incident sitting above the surface, there are many unrecorded near misses submerged below the surface.
Capture the near misses and act on the causes in order to reduce or eliminate actual incidents occurring.
In most companies an incident results in a rigorous Incident Report, an inquest into why the incident happened and actions to prevent it from happening again.  
All well and good and very necessary

Preamble
I have worked in many companies that I would describe as extremely safety focused and yet they still have incidents.  The incident investigation will always reveal a cause.  Some causes are more obvious than others but there is always a preventable cause.  It may be human,  it may be equipment, it may be process.  But always preventable.

The government has apparently been particularly wary of slips in standards in the construction industry. Construction firms have been very badly hit by the downturn, and it is thought that some may have taken shortcuts in order to keep their costs down.

UK - Olympics project gears up for toughest year yet

The Olympic Delivery Authority has reaffirmed its commitment to health and safety at the start of what is likely to be the London 2012 project’s most challenging year.
ODA chairman John Armitt applauded the health and safety record of the project, which, so far, has notched up eight sets of a million hours worked without a reportable accident. However, he acknowledged that there are significant challenges ahead: "The number of workers and activity on site will reach their peak, with construction underway in every part of the Olympic Park. The logistical challenge is huge, with the workforce increasing to around 11,000 and deliveries increasing from the current rate of around 650 per day to around 1000 per day."

UAE pledges to ensure security, safety at nuclear facilities

The United Arab Emirates (UAE) pays high attention to security and safety at its planned nuclear facilities and will try to prevent any emergency and crisis, the official news agency WAM reported Tuesday, citing an official.

As more and more countries push forwards with nuclear facilities, designers and suppliers are set for rich rewards.  The old order is again being challenged by Asian powerhouse engineering exporters such as South Korea.  With 1,400 new nuclear plants forecast to be built by the year 2050 there is significant market potential.

Nuclear power provides a "nominally" clean source, but the real potential for catastrophic danger is in the operation, maintenance and design.

 
UK - New tower crane regulations

Following a number of high profile failures of tower cranes in recent years, the Health and Safety Executive (HSE) has announced its proposals for a conventional tower crane register which is expected to come into force on 6 April 2010. There are around 1800 conventional tower cranes thought to be operating on construction sites in Britain with around 1300 in use at any one time. Given those figures there will be a substantial number of construction businesses affected by the proposed new requirements.

Preamble
All too often, people switch off the safety mindset when they leave their place of work .  Whether at work or not, there is a need to think and act safely, always.  Because of the high safety focus in most industrial work situations, there is a tendency for people to “switch off”, when not in the work environment.   It is very common for the mindset to be  “at work  = high danger, non work places = low to zero danger”.  Of course the reality is that danger is lurking everywhere and we need to think and act accordingly.

China - Safety in numbers

In July 2009 a paper was published reporting on a tragic industrial accident in a factory in China. It described lung injuries in seven women who worked processing polystyrene boards for use in the printing and decorating industry. The women were reported to have suffered from conditions affecting the lung and heart. Although five of the women were reported to be in a stable state at the time of writing, two others died of respiratory failure.
